NURSE FAMILY PARTNERSHIP
The Nurse Family Partnership program is a voluntary, home visiting program for first time, low-income pregnant women and their babies.
Nurses provide education, support and guidance regarding pregnancy and maternal health, child health and development, parenting, the mother’s life course development, and identifying and using family and community supports. Visits begin in the first or second trimester of pregnancy, and continue on a weekly or bi-weekly basis until the child turns 2 years old.
How can I enroll my patient in the Nurse Family Partnership program?
Eligibility criteria include first time pregnancy prior to 28th week gestation, and Medicaid-eligibility. To find out more about the program in your area, or to make a referral, contact the NFP Supervisor in your area. Click here for a list of NFP Supervisors in the state of Louisiana.
HEALTHY START
There are four Healthy Start programs that currently offer case management in the state of Louisiana. Each has different qualifying criteria for patients. To have a Case Manager assess your patient’s individual situation, contact the Healthy Start program in your region from the following list:
