Visit the Louisiana Department of Health and Hospitals website or call the Referral Assistance Hotline at 1-877-455-9955.
It offers a listing of dentists by region that accept Medicaid. As noted on the website, we cannot guarantee that all of these dentists are currently accepting new patients. Please ask if they are accepting new patients before making an appointment.
Your patient will need to bring the dental provider a referral from you using the BHSF Form 9-M. She must bring an original completed form to a participating dentist prior to receiving any dental services covered by Medicaid.
