Breastfeeding

Where can I find information about breastfeeding to offer my patient?
The La Leche League website has lots of useful information for moms-to-be. Click on the link below for answers about breastfeeding and frequently asked questions.

The Partners for Healthy Babies Helpline at 1-800-251-BABY offers a complete listing of lactation resources statewide. Our helpful operators can assist you in finding a lactation consultant or resource for your patient.

Community Case Managers

How can I get a Community Case Manager assigned to my patient?
There are four Healthy Start programs that currently offer case management in the state of Louisiana. Each has different qualifying criteria for patients. To have a Case Manager assess your patient’s individual situation, contact the Healthy Start program in your region from the following list:

Nurse Family Partnership

What is the Nurse Family Partnership?
The Nurse Family Partnership program is a voluntary, home visiting program for first time, low income pregnant women and their babies. Nurses provide education, support and guidance regarding pregnancy and maternal health, child health and development, parenting, the mother's life course development, and identifying and using family and community supports. Visits begin in the first or second trimester of pregnancy, and continue on a weekly or bi-weekly basis until the child turns 2 years.

How can I enroll my patient in the Nurse Family Partnership program?
Eligibility criteria include first time pregnancy prior to 28th week gestation, and Medicaid-eligibility. Currently, the program is targeting first time teenage mothers (under age 17), but older mothers also may be eligible. Smoking Cessation

How can I help my patients stop smoking?
By Becoming a Fresh Start Family smoking cessation provider/facility!

Make Yours a Fresh Start Family (MYFSF) is a perinatal smoking cessation training program. Women in the perinatal period are more likely to be willing to quit smoking, so it is a perfect time for a provider-initiated intervention. When you join our training program, you will receive free training for yourself and your staff, as well as materials and ongoing program support. Our training program will help you learn to counsel smokers to quit in 5 minutes or less, so this won’t add a lengthy time burden to your patient visits. Plus, qualifying facilities have the opportunity to earn points that can be redeemed for additional educational materials or even monetary stipends, where applicable.

Medicaid Providers

How can I help my patient find a provider who accepts Medicaid?

It offers a listing of providers by region that accept Medicaid, as well as information on other resources such as pharmacies and dentists who accept Medicaid.

Medicaid Dentists

How can I help my patient find a dentist who accepts Medicaid payment for the Expanded Dental Services for Pregnant Women?
Click on this link to the Louisiana Department of Health and Hospitals website.

It offers a listing of dentists by region that accept Medicaid. As noted on the website, we cannot guarantee that all of these dentists are currently accepting new patients. Please ask if they are accepting new patients before making an appointment.

Your patient will need to bring the dental provider a referral from you using the BHSF Form 9-M. She must bring an original completed form to a participating dentist prior to receiving any dental services covered by Medicaid.

Transportation Services

How can I help my patient find transportation to health services?
Transportation for non-emergency related medical visits can be arranged by calling one of the Regional Dispatch offices listed below. You or your patient should call the dispatch office 48 hours before the appointment. Same day transportation can be scheduled only when absolutely necessary.
